I am writing to tell you that you are required to attend a disciplinary meeting on Monday at 9am which is to be held in my office.
At this meeting the question of disciplinary action against you, in accordance with the Company Disciplinary Procedure, will be considered with regard to:
Fitting a customer’s toilet with a door handle.

The possible consequences arising from this meeting might be:
Verbal or written warning.
You are entitled, if you wish, to be accompanied by another work colleague or a trade union representative.
PS – I appreciate that you’re only an trainee plumber Anja, but just because the shop assistant at Wickes said that the only handles they have in stock are door handles, it doesn’t mean that you can use them on a customer’s toilet. The customer in question is very upset about what you’ve done and is refusing to pay for the job unless we give them a doorstop to hold up the toilet seat.
