I am writing to tell you that you are required to attend a disciplinary meeting on Monday at 9am which is to be held in my office.
At this meeting the question of disciplinary action against you, in accordance with the Company Disciplinary Procedure with regard to the apprentice electrician position, will be considered with regard to:
Wiring a plug socket using bits of dry wood

You are entitled, if you wish, to be accompanied by another work colleague or a trade union representative.
PS – I know that I told you to use your initiative when you told me that there were no more plugs available, but I had hoped that you would have taken that to mean ‘go to the shop and buy a new one’ rather than almost setting fire to the place.
