Say where it happened (which room or place):
Bromsgrove Rd. Redditch
Say how it happened (include the cause if you can):
Well, I narrowly avoided a collision with the driver of a Ford Escort van who proceeded to perform the ‘wanker’ sign at me before driving off at speed. I know that my articulated lorry isn’t really supposed to be driven at high speed off the motorway, so I tried to catch up with the Escort van whilst remaining cautious of oncoming traffic. Unfortunately, as I passed him whilst beeping the truck’s horn and flipping the bird at him through the window, a car pulled out in front of me in the opposite lane so I was forced to cut back in to the left hand side. It would appear that I overestimated my lorry’s Anti-lock brake system and proceeded to slide about wildly across both lanes spilling my payload of roughly four tonnes of King Edward potatoes everywhere. As you can see from the picture attached, the person that I had the altercation with is the person that had the accident.
Say if the person who had the accident suffered any injury:
Hard to say as he was covered by roughly 2000 spuds that the emergency services had to dig through to get him out.

Complete this box, if the accident is reportable under the Reporting of Injuries, Diseases and Dangerous Occurrences Regulations 1995 (RIDDOR).
How was it reported?: We only found out about this when the crisp factory called to ask where the f*ck their order for 30,000 spuds had got to. When we said we didn’t know they then asked how the f*ck we’ve managed to lose a lorry laden with 30,000 spuds.
